Injuries Are Falling Nationally — So Why Are Warehouse Rates Still Double the Average?

The National Picture: A Genuine, Record-Setting Decline

The headline numbers from the U.S. Bureau of Labor Statistics are unambiguously good. Private industry employers reported 2.5 million nonfatal workplace injuries and illnesses in 2024, down 3.1 percent from 2023 — the lowest number of employer-reported injuries and illnesses for this data series going back to 2003.2

The total recordable case rate was 2.3 cases per 100 full-time equivalent (FTE) workers, down from 2.4 in 2023, the lowest TRC rate for this data series going back to 2003.1

Importantly, the improvement was broad. The retail trade, manufacturing, and real estate and rental and leasing sectors also saw decreases in their TRC rates in 2024, and no industry sector had a TRC rate increase in 2024.3 Even traditionally high-risk health care improved, with health care and social assistance, which historically posts among the highest injury and illness rates, seeing its case rate fall to 3.4 cases from 3.6.1

But there's an important caveat for interpreting the topline figure: the decrease in 2024 was driven by a 26.0-percent drop in illness cases to 148,000, in turn due to a 46.1-percent decrease in respiratory illness cases to 54,000.2 In other words, much of the gain reflects the continued fading of COVID-era respiratory illness reporting rather than a uniform decline in physical injuries. Indeed, the 2.2 injuries per 100 FTEs in 2024 was unchanged from 2023.1

The Warehouse Gap: Roughly Double the National Average

Against that backdrop, warehousing stands out. While warehouses experience 4.8 cases per 100 workers, the overall private industry average sits at just 2.3 per 100 workers.11 That gap is structural, not a one-year anomaly — BLS reports that injury rates in warehousing and transportation are consistently higher than the national average: 4.8 cases per 100 full-time workers in 2022, compared to 2.7 for all private industries.19

State-level data shows the disconnect even more starkly. In New York, between 2017 and 2022 the rate of warehouse worker injuries more than doubled from 3.6 to 8.8 cases per 100 workers, including a 20 percent jump from 2021 (6.5) to 2022 (8.8).17 Severity is rising too: injuries are getting more severe, with the share requiring missed days of work or job transfer now at 89 percent, compared to 60 percent in 2017.17 And warehouse workers are injured more frequently than workers in any other industry in New York State, with warehousing ranking as the industry with the highest rate of injury among all industries in the state.17

Part of the story is explosive growth outpacing safety investment. A 2021 Department of Labor Office of Inspector General report showed warehouse-related injuries nearly doubled, from around 42,500 to over 80,500 cases, while the number of warehouse facilities grew just 14%.16 The rising national tide of safety gains is simply not reaching the warehouse floor.

What's Driving Warehouse Injuries

Regulators have pinpointed the specific hazard drivers. The GAO found that three major hazards — falls, slips, and trips; objects and equipment; and overexertion/bodily reaction — were the leading causes of most injuries and illnesses in general warehousing and last-mile delivery services.41 This mirrors the national pattern, where over the 2-year 2023-2024 period the highest number of DART cases were caused by overexertion, repetitive motion, and bodily conditions at 946,290, followed by contact incidents with 860,050 cases.1

The risks cluster in predictable places. Slips, trips, and falls account for 27% of all non-fatal warehouse injuries reported to OSHA.23 Forklifts are a second major driver — industry analysis indicates that 35,000 to 62,000 forklift injuries happen each year in the United States alone.11 Notably, OSHA estimates that 70% of forklift accidents in the United States could be prevented with stricter training policies.11

OSHA has responded directly. On July 13, 2023, OSHA announced a National Emphasis Program intended to remain in place for three years, focused on identifying, reducing or eliminating hazards relating to warehouse and distribution center operations.42 Inspections concentrate on powered industrial vehicle operations, material handling and storage, walking-working surfaces, means of egress, and fire protection, and also take into account heat and ergonomic hazards.44 Crucially, the program was driven by injury rates that are nearly double the national average and the growing number of employees.46

Why Visual Communication Is a High-Leverage Fix

Each of these incident drivers maps cleanly onto well-established visual-communication remedies — a comparatively low-cost intervention against a problem that automation alone has not solved. A recurring theme in OSHA citation patterns is that gaps stem from systemic gaps — not always blatant "safety violations," but often lack of training, complacency, or missing documentation (e.g., training records, PPE logs, hazard communication signage).22

Hazard labeling and HazCom. Hazard communication is consistently among the most-cited standards. In FY2025, OSHA's top 10 most frequently cited standards included Powered Industrial Trucks (29 CFR 1910.178), Respiratory Protection, and Eye and Face Protection.27 For chemicals, OSHA's Hazard Communication Standard, 29 CFR 1910.1200, aligns with the Globally Harmonized System (GHS), which requires elements on labels such as pictograms, signal words, hazard statements, precautionary statements, product identifiers, and supplier information.36

Standardized hazard signs. OSHA's accident-prevention sign standard, 29 CFR 1910.145, covers the specifications for accident prevention signs and tags but does not mandate a single specific sign design, allowing employers to use either older ANSI Z35 standards or the contemporary ANSI Z535 series.37 The value of the modern approach is comprehension: words take time to read, symbols are instant — ANSI Z535 provides guidelines for safety pictograms covering hazard alerts, mandatory actions like wearing PPE, and prohibitions, helping cross language barriers and improving reaction times.37 ANSI standardizes color so hazards register pre-consciously — for example, yellow signals caution for minor to moderate injury risk, such as tripping hazards.36

Exit-route clarity. Blocked or unmarked egress is a common warehouse citation. OSHA requires that if the direction of travel to the exit is not immediately apparent, signs must be posted along the exit access indicating the direction to the nearest exit, and the line-of-sight to an exit sign must clearly be visible at all times.31 Pictograms are permitted: where approved by the authority having jurisdiction, NFPA 101-2009 allows the use of pictograms in compliance with NFPA 170 in lieu of, or in addition to, signs with text.32

PPE and ergonomic-lifting reminders. Because overexertion is the single largest injury category and the NEP explicitly targets ergonomics, point-of-task visual reminders for proper lifting and required PPE address the highest-frequency driver. The logic is well documented: workers who understand why specific equipment protects them are more likely to use it consistently than those who see it as an arbitrary requirement.29

Conclusion

National injury data is genuinely improving, but warehousing remains roughly double the private-industry average — and in some states the gap is widening rather than closing. The hazards are well-characterized: slips/trips/falls, musculoskeletal overexertion, and forklift incidents, the very risks OSHA's warehouse NEP was built to attack. A comprehensive, ANSI Z535–aligned visual-communication program — hazard labeling, GHS-compliant HazCom, clear egress marking, and point-of-task PPE and lifting reminders — targets each of those drivers directly, supports compliance with the standards OSHA cites most often, and does so at a fraction of the cost of automation or facility redesign. For an industry where the safety tide has yet to come in, it is among the highest-leverage levers available.
